companies to participate at our Fall
Meeting by sponsoring a table at the
vendor fair, sponsoring our breakfast,
sponsoring our lunch, sponsoring a
workshop. or sponsoring our door
prizes.

return it with the fee by September 16,
2011.
Space is limited and vendor booth
assignments will be made until all
spaces are filled. Make checks payable
to the North Carolina Point of Care
Network and mail to the address
located on the vendor form.

space. Parking is free.

Entrance or the Patient Tower
Entrance.
