The North Carolina Point of Care Network would like to invite POC companies to participate at our Fall Meeting by sponsoring a table at the vendor fair, sponsoring our breakfast, sponsoring our lunch, sponsoring a workshop. or sponsoring our door prizes. 

Please click on one of the links below for the application, complete the vendor information and return it with the fee by September 16, 2011.  Space is limited and vendor booth assignments will be made until all spaces are filled.  Make checks payable to the North Carolina Point of Care Network and mail to the address located on the vendor form. Application [PDF] or [WORD] file.

Print the flyer above and use the directions on page 2 or:

  • Go to www.rexhealth.com

  • Select “Raleigh (Rex Hospital)” from Locations

  • Select “Driving Directions” at the bottom of the page for campus map and detailed directions to Rex Hospital.

  • You may park in any Visitor Parking space.  Parking is free.
    You may enter through the Main Entrance or the Patient Tower Entrance.

For the meeting room location:

  • Select  “Building Map” at the bottom of the page

  • The meeting will be held in “Assembly Room 1209” on the first floor which is shown on the building map.

  • You will enter the building on the second floor.
